National Health Service
This fact sheet has been written to explain the role of UK health services, the National Health Service (NHS), to newly-arrived individuals seeking asylum. It covers issues such as the role of GPs, their function as gatekeepers to the health services, how to register and how to access emergency services.

Requirements to register:

·
GMS1 Form
·New Patient Questionnaire - This is to help us understand your medical history
·1 form of photographic identification e.g. Passport, Driving Licence etc
·Proof of address within the last 3 months e.g. utility bill, bank statement etc

Please download and complete the GMS1 form and New Patient Questionnaire by clicking on the links below. Both these forms are also available to collect at our Reception desk which you can complete at your own leisure.

We advise that you check the information on this website about the practice and the vast number of services available nationally and locally which form part of your NHS. Many other support organisations exist in the voluntary sector.

When you join us:
We are a group practice and you may see any doctor by choice, subject to their availability. During busy periods or if the doctor is on annual leave, please be prepared to see one of the other GP’s.

Once you have been registered at the surgery you will require a New Patient Health Check (unless you are under the age of 16) this is usually booked at the time of your registration.
Please note: the registration process can take 5-7 working days

The New Patient Health Check Consultation will include:
•Height & Weight - BMI checks
•Blood Pressure Check
•Lifestyle & Dietary advice
•Advice on exercise
•Smoking status check and advice
•Cervical screening status check
•Health history review
•Family history and risk
