Sydenham House Group Practice welcomes new patients. Based in the Boulevard, West Hull, our fascinating building was originally constructed in 1876. With a long tradition in hosting general practices on this historic site, there has been a quantum leap in the range and quality of services available. Yet the traditional caring values have continued through the generations.
All consultations are by appointment, Monday to Friday 8.30am to 6.30pm. We also offer a Saturday surgery from 9am to 1.30pm.

ACCESS FOR DISABLED
Sydenham House has ground level access for the disabled with special toilet facilities and loop induction for those using hearing aids. There is also a short-term car space for dropping-off and collection only.
In the absence of a lift, arrangements are made for disabled patients to be seen on the ground floor - even if their doctor normally consults on the first floor. Advanced notice is appreciated so that the facilities can be prepared.
HOUSE RULES
For the comfort of patients and for health & safety reasons, with the exception of guide dogs, we are sorry that we can not allow animals into the building - including the foyer. Alcohol may not be consumed on the premises and it is against the law to smoke on the premises
The Practice operates a zero tolerance approach to the abuse of staff, other patients or visitors. Persons who are abusive may be removed from the list of patients. In cases of threatened violence or actual violence police involvement and removal is automatic and immediate. CCTV recording is in operation for the protection of staff and patients.
PHARMACY
Various pharmacies are available in the area. Some offer special services like late night opening and often can organise your repeat prescription ordering and delivery. In participating Pharmacy Scheme premises, you can also ask for basic health advice.
A SUMMARY OF KEY SERVICES
Asthma Clinic
In our special asthma clinics, patients diagnosed with the condition are offered advice, monitoring and treatment - which means that their condition is well-managed. With doctor oversight, the clinics are nurse-led. Patients may still consult with their doctor at any time.
Cervical Cytology
Eligible female patients are invited for smears. Our staff ensure that there is a meticulous attention to detail at all stages. We operate a full call and recall system for cervical cytology.
Choose & Book
If your doctor has recommended a referral for you to see a specialist doctor or clinic, he will have discussed choices available with you and will direct you to our trained reception staff who will book an appointment with you there and then. This service can be booked by the patient. You will be given a password to access the system - either at home or wherever a computer is available.
Chronic Kidney Disease (CKD)
Patients with this condition are offered appointments with our practice nurse who is fully-trained and works with our doctors to provide a specialist clinic.
Diabetes
Once diagnosed, patients may join our diabetes programme. This enables us to offer advice, monitoring and treatment so that their condition is well-managed. With doctor oversight, the clinics are nurse-led. Patients may still consult with their doctor at any time.
Doctor Consultations
This is a group practice and patients should be prepared to see any doctor if their case is urgent. However, they may select a doctor or doctors at their discretion and see that doctor or doctors, providing there are appointments available.
NHS Health Check
Patients between the ages of 40 and 74 who are not on any vascular disease register are being invited to be part of our NHS Health Check programme.
Our nurse-led clinics offer short initial assesments. Patients will have access to healthy eating advice and general lifestyle tips on how to stay in good condition. Simple tests are conducted and a risk assessment carried-out. This is a valuable pro-active programme which can have extremely beneficial effects.
Hypertension
Patients with high blood pressure are offered appointments to have their condition monitored. Advice is given and if necessary internal referrals made to a GP at the practice.
Hypothyroidism
The practice offers routine tests for patients with hypothyroidism - with GP follow-up if required.
Ischemic Heart Disease (IHD)
Appropriate simple tests are offered to patients attending our IHD clinic. They are also offered a follow-up appointment shortly afterwards to discuss the results and receive advice. A register and recall system is in operation to ensure efficient monitoring so that patients can effectively monitor and manage their condition
Learning Disabilities
This embraces a wide range of conditions and the aim of our clinics is to manage the general health and well-being of patients. The nurse-led clinic offers advice on diet, exercise and lifestyle.
Mental Health
Mental Health embraces a wide range of conditions and affects a very large number of people at some time in their lives. We do not unduly label people and our routine service aims to ensure the well-being of patients with general health monitoring and advice.
Nursing
Whilst our practice nurses are trained to carry out a variety of specialised tasks such as asthma diabetes and IHD clinics, the department also handles many basic and routine treatments such as dressings, immunisations ear syringing and ECGs. According to their level of training, some of these duties are carried-out by health care assistants or specialist phlebotomists
Smoking Cessation
In addition to other smoking cessation services in the locality, patients can access one to one advice and support from our practice nursing department. Bookings for our practice nurse can be made at reception. Other services can be accessed on-line or by self referral.
Vaccination & Immunisations
The practice offers a very wide range of vaccination and immunisations - for children and adults. This includes seasonal flu and travel vaccines. We are a designated yellow fever centre.
